What is poly-L-lactic acid?

Poly-L-lactic acid (PLLA) is a biocompatible polymer that the body breaks down. It has been used in medicine since the 1960s, including in resorbable sutures. In Sculptra, PLLA is supplied as a powder that is reconstituted in sterile water and injected deep subdermally, most often in the face.

How it works in treatment

PLLA breaks down gradually over 9 to 12 months and signals fibroblasts to build new collagen, mainly type I. That is the structural collagen that gives skin firmness and thickness. The process is gradual. Sculptra is therefore given in 2 to 3 sessions 4 to 6 weeks apart, and the final result is judged after 6 months.

Where it is used

PLLA is used mainly for large-scale volume restoration in cheeks, temples, and the lateral mid-face in patients with general volume loss. Point injections with hyaluronic acid become unrealistic or short-lived in those cases. PLLA is also used on the décolletage, thighs, and for laxity after weight loss.

Results and safety

Results last up to 25 months once full collagen production is established. Three critical points: reconstitution time (at least 24 hours), correct injection depth, and the 5×5×5 post-treatment massage (5 minutes, 5 times a day, for 5 days) to prevent nodules. PLLA cannot be dissolved like hyaluronic acid. Outcomes depend entirely on the injector's training and routine.
