Not because it delivers the most dramatic result in an afternoon, but because it does something no filler in the world can: it gets your body to build new collagen, entirely on its own.

It is a simple but powerful idea. Instead of filling what has been lost, we introduce a substance that awakens the skin's own repair mechanisms. The result comes gradually, week by week, and it looks like you. Not like a treatment.

What is Sculptra?

Sculptra is an injectable biostimulator containing poly-L-lactic acid (PLLA), a fully biodegradable substance that has been used in medicine for decades. PLLA is the same material found in absorbable surgical sutures, threads that the body breaks down naturally without leaving a trace.

In aesthetic medicine, Sculptra has been approved in the EU for over 20 years. It was originally launched to treat facial atrophy in HIV patients, a condition involving dramatic volume loss in the face. The results were so convincing that Sculptra quickly received a broader indication: facial skin rejuvenation in patients with age-related volume loss.

PLLA: not a filler

It is important to understand what Sculptra is not. It is not a filler. Fillers such as hyaluronic acid (Juvederm, Restylane, Stylage) fill mechanically. They add volume through a gel placed beneath the skin. The effect is immediate and visible straight away.

Sculptra works the opposite way. The PLLA microparticles are injected into the skin, and over the following weeks they stimulate the body's fibroblasts to produce new collagen. The PLLA particles themselves are completely broken down by the body within a few months, but the collagen that has formed stays. It is your own collagen, built by your own cells.

Think of it this way: a filler is like renting furniture for a room. Sculptra is like renovating the room. The furniture can be removed, but the new structure remains.


How does Sculptra work?

The science behind Sculptra is about harnessing the body's natural healing process. When the PLLA microparticles are injected into the dermis (the skin's middle layer), the body perceives them as a foreign material that needs to be dealt with. This triggers a controlled inflammatory response that sets off a chain of biological events.

Fibroblast activation

The fibroblasts, the skin's collagen-producing cells, are activated and begin working around the PLLA particles. They produce new type I and type III collagen, precisely the collagen types that give skin its firmness and elasticity. Over time, a network of new collagen forms that replaces the volume and structure that ageing has taken.

Gradual build-up

This is the key to why Sculptra looks so natural. The change does not happen overnight. It builds over 4 to 12 weeks, as the collagen network grows. Your friends and colleagues notice that you look rested and fresh, but they can rarely pinpoint exactly what has changed.

Lasting effect

The PLLA particles are completely broken down by the body within approximately 12 to 18 months. But the collagen that formed during that time remains. This means you have a result that is built on your own tissue, not on a product that is slowly disappearing.

Sculptra vs fillers and other biostimulators

One of the most common questions I receive is: Should I choose Sculptra or fillers? And the answer is that it depends entirely on what you want to achieve. Here is an honest comparison.

Sculptra vs hyaluronic acid fillers

Fillers such as Juvederm, Restylane and Stylage provide immediate volume. You see the result straight away in the mirror. They are perfect when you want to fill a specific wrinkle, add volume to the lips or define the cheek contours. The effect lasts 6 to 18 months depending on the product and placement.

Sculptra provides no immediate volume. You do not see the final result until several weeks to months later. But Sculptra builds new collagen that delivers a general improvement in skin quality, firmness and elasticity. The result lasts up to 2 years.

In short: fillers compensate for volume loss. Sculptra treats the cause of volume loss.

Sculptra vs Profhilo

Profhilo is a biostimulator based on highly concentrated hyaluronic acid. It stimulates both collagen and elastin, and above all delivers fantastic moisture balance and radiance. Profhilo is injected using the BAP technique (5 points per side) and is particularly effective for skin that feels dehydrated, lax and dull.

The difference from Sculptra? Profhilo works more with moisture and elasticity, while Sculptra builds volume and structure through collagen stimulation. They complement each other excellently, and many of my clients combine them.

Sculptra vs HarmonyCa

HarmonyCa is a hybrid: it combines hyaluronic acid filler with calcium hydroxylapatite microspheres. This means you get immediate volume and long-term collagen stimulation in the same treatment. HarmonyCa is excellent for providing a lifting effect in the mid-face and along the jawline.

Sculptra lacks the immediate filling effect, but on the other hand provides a more diffuse, generalised collagen build-up. HarmonyCa is more targeted, Sculptra more comprehensive.

Sculptra vs Radiesse

Radiesse contains calcium hydroxylapatite and, just like HarmonyCa, provides both immediate filling and collagen stimulation. Radiesse is popular for the jawline and cheeks. Sculptra builds more collagen over time but lacks the immediate filling effect.

Sculptra vs Rejuran

Rejuran works with PDRN (polynucleotides) and focuses on cellular repair, skin quality and anti-inflammation. Rejuran is fantastic for fine lines, skin texture and dark circles. Sculptra targets volume loss and skin laxity more directly. They combine excellently: Rejuran for skin quality, Sculptra for structure.


Treatment areas

Sculptra is versatile but works best in specific areas where age-related volume loss and skin laxity are most noticeable.

Where Sculptra makes the greatest difference

Cheeks and cheekbones. The most common treatment area. With age, the cheeks lose their natural fat and collagen, creating a sunken, tired appearance. Sculptra builds volume from within and creates a soft, natural contour.

Temples. Hollow temples age the face more than most people realise. Sculptra gradually fills the area and gives the face a more youthful balance.

Jawline. Skin laxity along the jawline, sometimes called jowls, is one of the most common signs that collagen support has weakened. Sculptra strengthens the structure from below and creates a more defined jawline.

Nasolabial folds. The deep folds from nose to mouth corner improve when cheek volume is built up from within. Sculptra does not treat the folds directly, but the underlying volume loss that causes them.

Neck and decolletage. Sculptra can be used to improve skin quality and firmness on the neck and chest, areas that often show signs of ageing early.

Where Sculptra is not used

Sculptra is not suitable for all areas:

  • Lips: Sculptra should never be injected into the lips. For lip augmentation, we recommend hyaluronic acid fillers.
  • Forehead: The forehead area is better treated with Botox or fillers.
  • Nose: Nose filler with hyaluronic acid is the right choice for non-surgical nose correction.
  • Directly under the eyes: Tear trough treatment is done with specific fillers, not Sculptra.

How the treatment works at Dibélle

Consultation

Every Sculptra treatment begins with a thorough consultation. I analyse your skin, discuss your goals and create an individual treatment plan. We talk about how many sessions you are likely to need, which areas take priority and what you can realistically expect.

During the consultation, I also assess whether Sculptra truly is the right choice for you, or whether another treatment would suit you better. Honesty is more important than selling a treatment.

By law, a 48-hour reflection period applies before undergoing an aesthetic injection treatment. This means you are never treated at the same appointment as your initial consultation.

Preparation

On treatment day, we begin by cleansing the skin. I mark the points where Sculptra will be injected based on your individual plan. A medical numbing cream is applied and left to work for approximately 20 to 30 minutes. Sculptra is mixed with sterile water and lidocaine (local anaesthetic) before injection, making the treatment comfortable.

Injection

Sculptra is injected with a thin needle or microcannula, depending on the treatment area and technique. I work in layers and distribute the product evenly in the subcutaneous layer. The treatment takes approximately 30 to 45 minutes depending on how many areas are being treated.

During the injection, I massage the area continuously to ensure the product is distributed evenly. Uneven distribution can lead to lumps, and correct technique is crucial.

Immediately after

Immediately after the treatment, I massage the entire treated area thoroughly. You receive instructions on the important at-home massage (more on that under Aftercare). You can return to your daily routine straight away, but may have mild swelling and redness lasting a couple of days.

Recovery and aftercare

Recovery after Sculptra is mild compared to many other aesthetic treatments, but aftercare plays a decisive role in the final result.

The 5-5-5 rule

The most important part of Sculptra aftercare is massage. We call it the 5-5-5 rule:

  • 5 times a day
  • 5 minutes each time
  • 5 days after treatment

The massage ensures that the PLLA particles are distributed evenly in the tissue and reduces the risk of unevenness or lump formation. I show you exactly which technique to use during your visit.

Day by day

Day 0 (treatment day): Mild swelling and possible redness in the treatment area. This is completely normal. Begin the massage that same evening.

Day 1 to 2: Swelling may increase slightly before subsiding. Avoid intense exercise, sauna, alcohol and direct sun exposure.

Day 3 to 5: Swelling has settled for most people. Continue with the 5-5-5 massage. You can resume your normal skincare routine and makeup.

Day 5 to 7: The skin looks normal. The initial volume you saw immediately after treatment (from the fluid in the mixture) has gone down. Do not worry, this is expected. The real result begins to build over the coming weeks.

Important aftercare tips

  • SPF 50+ daily for at least 2 weeks after treatment
  • Avoid alcohol for the first 24 hours
  • Avoid sauna and intense exercise for 48 hours
  • Avoid retinol and strong acids for 48 hours
  • Sleep with your head slightly elevated on the first night
  • Drink plenty of water to support collagen production

How many treatments are needed?

Sculptra is a cumulative treatment. Each session lays the foundation for the next, and the full result requires several rounds.

Initial course

Most people need 2 to 3 treatments at 4 to 6 week intervals. The number depends on how much volume loss and skin laxity needs to be addressed. Younger patients with mild loss may manage with 2 sessions, while older patients with more pronounced volume loss may need 3 or in rare cases 4.

Timeline

Week 0 to 2: Initial swelling has settled. No visible improvement yet.

Week 4 to 8: Collagen production is underway. You start to notice that the skin feels firmer.

Month 2 to 3: Visible improvements. Cheeks look fuller, the skin has better resilience.

Month 3 to 6: Full effect. The newly produced collagen network has matured and delivers maximum improvement.

Maintenance

Many choose to have a maintenance treatment after 12 to 18 months to extend and build on the result. The maintenance treatment usually requires just one session.


Results and longevity

What makes Sculptra unique among aesthetic treatments is its longevity. The result can last up to 2 years, and in some cases even longer. This is because the treatment's effect does not depend on a product that slowly breaks down (like filler), but on the collagen your body has built.

The PLLA particles break down completely within 12 to 18 months. But the collagen network they stimulated remains. Over time, the skin naturally continues to age, and the newly formed collagen will also gradually diminish. But the starting point is better, you have essentially turned back the clock.

Most people find that the result is clearly visible for 18 to 24 months after the last treatment, with a gradual fading thereafter.


Who is Sculptra suitable for?

Ideal candidates

Sculptra is right for you if you:

  • Have noticeable volume loss in cheeks, temples or jawline
  • Want gradual, natural rejuvenation (not a sudden "new face")
  • Prefer long-lasting results over frequent follow-ups
  • Have skin laxity that does not require surgery but needs more than skincare alone
  • Want to combine with other treatments for a complete result
  • Are 35+ and beginning to notice your face losing its structural support
  • Want to invest in long-term collagen building

Sculptra is not suitable if you:

  • Are pregnant or breastfeeding
  • Are seeking an immediately visible result (then fillers are a better choice)
  • Want lip augmentation (use hyaluronic acid instead)
  • Have an active infection in the treatment area
  • Have a known hypersensitivity to poly-L-lactic acid
  • Have a severe autoimmune condition (discussed at consultation)

Frequently asked questions

Does Sculptra hurt?

Sculptra is mixed with lidocaine (local anaesthetic) before injection, and we always apply numbing cream in advance. Most people find the treatment entirely manageable, a pressing or tingling sensation rather than pain. The discomfort is brief. At Dibélle, we always prioritise your comfort and adapt the numbing to your needs.

Can I see results after one treatment?

You will notice some fullness immediately after treatment, but this is due to the fluid in the Sculptra mixture and not collagen. This initial volume settles within a few days. The real result, new collagen that provides natural fullness and firmness, develops gradually over 4 to 12 weeks. Most people see the clearest improvement after 2 to 3 sessions.

What are the risks of Sculptra?

Sculptra has a well-documented safety profile with over 20 years of clinical use in the EU. The most common side effects are transient: swelling (2 to 5 days), redness, tenderness and in some cases bruising at the injection points.

The most discussed risk is granuloma formation, small, firm nodules beneath the skin. With modern technique and proper aftercare (the 5-5-5 massage), this risk is very small. Correct dilution of the product, proper injection depth and thorough massage during and after treatment further minimise the risk.
