For years, you had to pick a side: filler or biostimulator. Either immediate volume, or slow collagen build-up. Since 2021, there is a third option. HarmonyCa is one of the few products on the market that does both jobs in the same syringe, and the result holds longer than most hyaluronic acid fillers.
At Dibélle on Kullagatan 40 in Helsingborg, we have worked with HarmonyCa since the product arrived in Sweden. It has quickly become one of our favourites for cheekbone volume and jawline definition, especially with clients between 35 and 55 who want something more lasting than traditional filler but are not quite ready for a pure biostimulator.
In this guide, I walk through how HarmonyCa works, which areas it is made for, how it compares with Radiesse and Sculptra, and what a treatment looks like with us.
What is HarmonyCa?
HarmonyCa is an injectable hybrid product from Allergan Aesthetics. It consists of two components blended in the same syringe: hyaluronic acid (HA), which acts as a carrier and provides immediate volume, and calcium hydroxylapatite (CaHA) in the form of microspheres that stimulate the body's own collagen production over several months.
The product exists in two variants on the European market:
- HarmonyCa with lidocaine: the standard variant with built-in local anaesthetic
- HarmonyCa: without lidocaine, for clients who cannot tolerate lidocaine
CE marked since 2021. At Dibélle, we use the variant with lidocaine as standard, because it makes the treatment experience significantly more comfortable.
HA + CaHA: two products in one
The hyaluronic acid in HarmonyCa is cross-linked, much like in a traditional HA filler. That is what provides the visible lifting effect you see immediately at the time of injection. But the HA gel breaks down gradually, which is where the CaHA microspheres come in.
The CaHA spheres in HarmonyCa have a diameter of 25 to 45 micrometres, the same size as in Radiesse. As the HA carrier is gradually absorbed by the body, the fibroblasts (the skin's collagen-producing cells) take over and start building new collagen around the spheres. The result is a treatment whose effect develops over several months.
Biocompatibility
CaHA is the same mineral found in bone and teeth. The body recognises it and breaks it down into calcium and phosphate ions. The hyaluronic acid breaks down enzymatically, just like any other HA filler. Neither material leaves permanent residues in the tissue.
How does HarmonyCa work?
The fact that HarmonyCa has two mechanisms of action means the result develops in two clear phases.
Phase 1: immediate lifting effect (week 0 to 4)
The very second HarmonyCa is injected under the skin, the HA carrier provides volume. This phase is identical to a traditional HA filler. You see the height in the cheekbones, the definition along the jawline and the contour straight away in the mirror when you leave the clinic.
Over the following weeks, the HA gel begins to break down, but the volume does not disappear, because phase 2 has already begun.
Phase 2: collagen build-up (week 4 to 18 months)
The CaHA microspheres remain in the tissue as the HA carrier is absorbed. Fibroblasts migrate to the spheres and start producing new type I collagen around them. This is the same collagen type that gives young skin its firmness.
In the months after treatment, the skin in the area gradually becomes firmer, and the result can even improve over time. Many clients report a continued improvement 8 to 12 weeks after treatment.
What makes HarmonyCa unique?
Other biostimulators do one thing. Sculptra builds collagen without any filling effect. Radiesse has CaHA in a water-based carrier that provides a lifting effect but is not cross-linked HA. HarmonyCa combines cross-linked HA with CaHA in the same syringe, which means the HA component is theoretically partly reversible if the result needs adjustment with hyaluronidase during the first few months.
That is an important distinction. Reversibility is one of the major advantages of HA fillers, and HarmonyCa keeps part of that advantage, which a pure CaHA product like Radiesse does not have.
HarmonyCa vs Radiesse vs Sculptra
These are the three biostimulator-related products we use most often at the clinic, and the question of which one suits you comes up regularly during consultations.
HarmonyCa vs Radiesse
Both contain CaHA microspheres of the same size. The difference lies in the carrier. Radiesse uses a water-based gel with sodium carboxymethylcellulose. HarmonyCa uses cross-linked hyaluronic acid.
In practice, that means:
- Consistency: HarmonyCa feels slightly softer in the tissue than Radiesse
- Reversibility: the HA component in HarmonyCa can be partly dissolved with hyaluronidase, Radiesse cannot be dissolved at all
- Application areas: both are excellent for cheekbones and jawline. Radiesse is more established for hand rejuvenation (FDA-approved for that indication)
HarmonyCa vs Sculptra
Sculptra is a pure biostimulator with no filling effect. It builds collagen gradually over 4 to 6 months, with no immediate volume change.
HarmonyCa gives volume right away and collagen stimulation as a bonus. The difference comes down to how fast the visible result appears, and what the products are fundamentally designed for:
- Want immediate lift and contour? HarmonyCa
- Want gradual rejuvenation of the whole face? Sculptra
- Want both, in combination? We often use both within the same overall plan
HarmonyCa vs hyaluronic acid filler
Regular HA filler lasts 6 to 12 months and fills out volume. HarmonyCa lasts 12 to 18 months and stimulates collagen on top of that. The difference in skin quality over time is significant, but HA fillers are more versatile and can be used in the lips, under the eyes and in the nose, areas where HarmonyCa should not be used.
Treatment areas
HarmonyCa has specific strengths. Knowing where it works and where it should not be used is crucial.
Where HarmonyCa works excellently
Cheekbones (mid-face): HarmonyCa's primary indication. The product provides lift and structure in the cheekbone area with long-lasting effect. The cross-linked HA component sits nicely against the bone, and the CaHA spheres continue to stimulate collagen build-up.
Jawline: the firmer consistency makes HarmonyCa ideal for sharpening the jawline and creating clearer definition between face and neck.
Nasolabial folds: the deeper folds from nose to mouth respond well to HarmonyCa. The immediate filling effect softens the folds, and the collagen stimulation provides a lasting result.
Temples: volume loss in the temples is an early sign of facial ageing. HarmonyCa fills out the area and gives a noticeably fresher overall impression.
Chin: for those who want a more defined chin profile, HarmonyCa can provide structure and projection without surgery.
Where HarmonyCa should NOT be used
Lips: the consistency is too firm for lip tissue. Use regular HA filler for lip augmentation.
Under the eyes (tear trough): the skin here is too thin and the product too opaque. HA fillers with a soft consistency are safer for this area.
Nose: the complex vascular supply in the nose makes CaHA-based products unsuitable.
How the treatment works at Dibélle
Consultation and reflection period
Every injection treatment begins with a consultation. We look at your facial proportions, discuss what you want to achieve and assess whether HarmonyCa is the right product for you. Sometimes we recommend a different biostimulator, or a combination of several products.
Under Swedish regulations, a 48-hour reflection period applies for injection treatments. Consultation and treatment take place on two separate occasions, at least 48 hours apart. The same treatment type can then be repeated within six months without a new reflection period. Read our guide on how to prepare before your visit.
Treatment day
The treatment begins with cleansing and a medical numbing cream that is left to work for 20 to 30 minutes. HarmonyCa also contains lidocaine in its own formulation, which makes the injection comfortable.
I mark the injection points based on the plan we made during the consultation. The marking is not cosmetic. It guides the exact depth and placement of every injection. For cheekbones and jawline, the product is placed deep, supraperiosteally against the bone, which is what creates the characteristic lifting effect.
The injection itself takes 20 to 40 minutes depending on the number of areas. I massage the treated area immediately after to distribute the product evenly.
Immediately after
You see the result straight away in the mirror. Mild swelling and redness are normal and may reinforce the impression during the first few hours. Most clients return to their daily routine in Helsingborg the same day.
Recovery and aftercare
HarmonyCa requires minimal downtime compared with more invasive procedures.
The first 24 hours: swelling, mild tenderness and possibly small bruises at the injection sites. Avoid alcohol, intense exercise, sauna and tanning beds.
Day 2 to 4: swelling subsides noticeably. You can apply makeup after 24 hours once the injection sites have settled. Any bruises start to fade.
Week 1: most side effects have disappeared. What you see in the mirror now reflects the final result of the HA phase.
Week 4 to 12: collagen stimulation produces visible results. The skin in the area gradually becomes firmer and the contour can even deepen.
Aftercare tips
- SPF 50+ daily for the first few weeks
- Do not sleep on the treatment area the first night
- Drink plenty of water, hydration supports collagen production
- Avoid facial treatments and laser for the first 2 weeks
- Do not massage the area yourself unless we have specifically asked you to
Results and longevity
HarmonyCa typically lasts 12 to 18 months, depending on treatment area, metabolism and lifestyle. That is longer than most HA fillers (6 to 12 months) but shorter than pure CaHA products like Radiesse in certain areas.
The good thing is that even once the product itself has broken down, the newly formed collagen remains. Many clients find that the skin looks better than before the treatment, even after the initial filling effect has faded. Maintenance treatments once a year build further on the result.
Who is HarmonyCa suitable for?
HarmonyCa is a good choice if you:
- Are looking for lift and contour in the cheekbones, jawline or chin
- Want immediately visible results combined with long-term collagen stimulation
- Prefer a filler that lasts longer than regular HA fillers
- Are 35 to 55 and want an investment with double action
- Value that the HA component is partly reversible with hyaluronidase
- Are not ready for a pure biostimulator like Sculptra, but want more than regular filler
HarmonyCa is not the right choice if you:
- Want lip augmentation
- Need treatment under the eyes
- Want a fully reversible treatment (the CaHA component cannot be dissolved)
- Are pregnant or breastfeeding
- Have an active infection in the treatment area
- Are under 18 years old
Frequently asked questions
Does HarmonyCa hurt?
With numbing cream and built-in lidocaine, most people find the treatment comfortable. A stinging sensation during the injection is normal but brief.
Can HarmonyCa be dissolved like a regular HA filler?
Only the HA component can be partly dissolved with hyaluronidase during the first months. The CaHA spheres cannot be dissolved and break down naturally over 12 to 18 months. This makes it extra important that the treatment is performed by an experienced injector.
How quickly will I see results?
You see volume and contour immediately after treatment. The real bonus comes 4 to 12 weeks later, when the collagen build-up provides a continued improvement.
How many sessions are needed?
For most people, one treatment is enough, with maintenance once a year. For more significant volume loss, we may recommend two sessions with 4 to 6 weeks between them.
What are the risks?
The most common side effects are mild and transient: swelling, redness, tenderness and bruising. Rarer side effects include nodules and asymmetry. Serious complications such as vascular occlusions are extremely uncommon with an experienced injector.
Can HarmonyCa be combined with other treatments?
Yes, we often combine HarmonyCa with Botox, Profhilo or Rejuran for a complete plan. We plan the timing carefully so the products do not interfere with each other.
