Few aesthetic treatments are as transformative — and as widely misunderstood — as fillers. In a world of filters and retouched images, it can be difficult to know what is achievable, what is reasonable, and what actually delivers a beautiful, natural result. This guide is written for you if you are curious, have perhaps been considering treatment for a while, or simply want to understand more about one of the most sought-after aesthetic treatments in Sweden.

What are fillers — and how do they work?

Fillers, or dermal fillers, are gel-based injections placed beneath the skin to add volume, contour, and soften lines and folds in the face. But to truly understand why they work so well, we need to talk about the body's own biology.

The role of hyaluronic acid in the skin

The vast majority of modern fillers are based on hyaluronic acid (HA), a substance that occurs naturally in your body. Hyaluronic acid is found in the skin, joint fluid, and eyes — and its primary function is to bind water. A single molecule of hyaluronic acid can bind up to 1,000 times its own weight in water, which explains why it is so central to the skin's volume, hydration, and elasticity.

The problem? The body's production of hyaluronic acid decreases with age. From as early as your mid-twenties, production begins to decline, and by the age of 50, approximately half of the natural reserves have been lost. The result is visible as:

  • Volume loss in the cheeks, temples, and under the eyes
  • Deeper lines and folds, particularly nasolabial folds (from nose to mouth)
  • Thinner, drier skin that loses its radiance
  • Altered proportions — the face can appear tired or flat

Fillers compensate for this loss. By injecting a gel formulation of hyaluronic acid, we can restore volume where it has diminished, build structure where it is needed, and give the skin a hydration boost from within.

Why hyaluronic acid is superior

There is an important reason why hyaluronic acid-based fillers dominate the market: safety. Since HA already exists in the body, the risk of allergic reactions is extremely low. Stylage

Stylage from Vivacy is a French premium brand that has become increasingly popular among experienced injectors. What makes Stylage unique is that it contains antioxidants (mannitol) in its formulation, which helps protect the hyaluronic acid from degradation and may extend its longevity.

Fillers can be used in a wide range of areas, and each area requires its own technique, product selection, and understanding. Here we walk through the most common treatment areas we perform at Dibélle.

Lips

Lip enhancement with fillers is one of our most popular treatments — and perhaps the one surrounded by the most myths. The truth is that modern lip fillers, when expertly administered, deliver a soft, natural, and proportionate result that enhances your own lip shape.

What we can achieve:

  • Volume increase — From subtle enhancement to more noticeable augmentation
  • Definition — Clearer lip border (vermillion border) and Cupid's bow
  • Symmetry correction — Most people have naturally asymmetric lips
  • Hydration and softness — Hyaluronic acid provides a natural sheen and moisture

Treatment details: Lip treatment usually takes 20–30 minutes including numbing. We apply a strong numbing cream 15–20 minutes before the procedure, and the filler also contains lidocaine. Most people describe the sensation as a light pressure rather than pain. Typically, we use 0.5–1 ml of filler per session. We strongly recommend starting conservatively — it is always possible to add more, but it is important to avoid overdoing it.

Longevity: 6–12 months depending on metabolism, lifestyle, and product.

Cheekbones

Cheekbones are the architectural foundation of the face. Over the years, we lose bone volume and fat pads in the mid-face, causing the face to sag downwards and appear flatter. Filler treatment in the cheekbones provides an elegant lifting effect that can rejuvenate the entire face without looking surgical.

Treatment details: Cheekbone treatment is often performed with a cannula (blunt needle) rather than a standard needle, which significantly reduces the risk of bruising. We inject deep, close to the bone, to create a stable foundation. The treatment takes 15–20 minutes per side, and we typically use 1–2 ml in total.

Longevity: 12–18 months. Cheekbones are one of the areas where fillers last longest, as there is relatively little movement in the area.

Jawline

A well-defined jawline gives the face structure and elegance. Jawline treatment with fillers has become enormously popular — and rightly so. It is a treatment that can make a dramatic difference to the profile, without surgery.

What we can achieve:

Treatment details: Jawline treatment requires a filler with high lifting capacity and structural support — we often use Juvéderm Volux or Restylane Lyft. The treatment takes 20–30 minutes and typically requires 2–4 ml of filler for a complete result. We often recommend spreading the treatment over two sessions to achieve the most natural result.

Longevity: 12–18 months.

Nasolabial folds

The so-called nasolabial folds — the lines running from the nostrils down to the corners of the mouth — are one of the most common signs of ageing. They are entirely natural and everyone has them to some degree, but deeper folds can create a tired or sad expression.

What we can achieve:

  • Soften deep lines — Filling from within lifts the folds
  • Fresher expression — The face looks more rested and vibrant
  • Preventive effect — Early treatment can prevent the folds from deepening further

Treatment details: We inject a medium-viscosity filler along the deepest point of the fold and build support from underneath. The treatment takes 10–15 minutes and typically requires 1–2 ml. The result is visible immediately but refines over the first 1–2 weeks.

Longevity: 9–15 months.

Chin

The chin plays a decisive role in the face's profile and balance. An underdeveloped or asymmetric chin can affect the proportions of the entire face. Filler treatment of the chin is a quick and effective method for achieving a harmonious profile.

Longevity: 12–18 months.

Tear trough

Dark circles and puffiness under the eyes are among the most common concerns we hear — and one of the areas that requires the most experience and finesse. The tear trough is the hollow area between the lower eyelid and the cheek, and when it lacks volume, a shadow is created that gives a tired, aged appearance.

What we can achieve:

  • Brighter eye area — Filling reduces the shadow that creates "dark circles"
  • Smoother transition — A more seamless line between the eyelid and cheek
  • Fresher expression — The eyes look more rested and vibrant

Treatment details: The tear trough is an advanced treatment area that requires extensive experience. The skin beneath the eyes is extremely thin, and the wrong product or incorrect placement can produce unwanted results. We use a very soft, low-viscosity filler and inject with a cannula for maximum safety. The volume is small — typically 0.3–0.5 ml per side. We are always conservative in this area.

Longevity: 9–12 months. Retreatment should be approached with caution.

Step 1: The personal consultation

Everything begins with a conversation. During the consultation — which usually takes 20–30 minutes — we listen to your wishes, analyse your face, and discuss realistic expectations. We look at:

Step 2: Treatment planning

Based on the consultation, we create a tailored treatment plan. This may include:

  • Selection of products and volumes
  • Priority order if multiple areas are to be treated
  • Timeline — sometimes we recommend spreading the treatment across several sessions
  • Budget and cost overview

Step 3: Preparation

Before the treatment:

  1. Numbing cream is applied to the treatment area (15–20 minutes for it to take effect)
  2. The skin is cleansed thoroughly and disinfected
  3. We mark injection points if needed
  4. We photograph the before image for documentation

Step 4: The treatment

The injection treatment itself is carried out with precision and calm. Abir works methodically, injecting in small amounts and assessing the result continuously. You have a mirror available throughout the entire process and can provide feedback in real time.

  • Technique: Needle and/or cannula, depending on the area

Step 5: Immediately after

Immediately following the treatment:

  • You get to see the initial result in the mirror
  • We provide you with detailed aftercare instructions (verbally and in writing)
  • We book any follow-up appointments
  • You can usually return to your daily routine straight away

Pain management — how much does it actually hurt?

The question we are asked most often: "Does it hurt?" The short answer: considerably less than most people expect.

Why it is more comfortable than you think

  1. Numbing cream — We apply a strong topical anaesthetic 15–20 minutes before the treatment. It numbs the skin surface and significantly reduces discomfort.
  2. Lidocaine in the filler — All premium fillers we use contain lidocaine, a local anaesthetic, built into the gel. This means each injection point is further numbed as the treatment progresses.
  3. Modern technique — The use of cannulas (blunt needles) in many treatment areas reduces discomfort compared to traditional needles.
  4. Experience — An experienced injector works quickly, safely, and with minimal tissue trauma.

Most patients describe the sensation as a light pressure or prick — not pain. Lip treatments are generally the most sensitive area, but even there the procedure is very well tolerated thanks to the dual numbing approach.


Results and longevity

What you can expect

The result of a filler treatment is visible immediately — but the initial result is not the final one. During the first few days, you may experience:

  • Swelling — Most common in the lips, can be noticeable for the first 24–48 hours
  • Slight asymmetry — Swelling rarely subsides symmetrically, but it will even out
  • Tenderness — Treated areas may be tender to the touch

The final result appears after approximately 2 weeks, once all swelling has subsided and the filler has integrated into the tissue. This is when we recommend a follow-up appointment to evaluate the result and make any fine adjustments if needed.

Factors that affect longevity

  • Metabolism — A high metabolic rate breaks down filler more quickly
  • Lifestyle — Intense exercise, smoking, and sun exposure can shorten longevity
  • Volume — More product tends to last longer
  • Product choice — Thicker, more cross-linked fillers generally last longer
  • Treatment area — Areas with a lot of movement (such as lips) break down filler more quickly

Maintenance treatments

Most of our clients choose to have regular maintenance treatments rather than waiting until the filler has completely dissipated. The advantages of this approach:

  • The result remains consistent and natural
  • Less product is needed at each session
  • The cost is spread over time
  • Studies suggest that regular treatments can stimulate the body's own collagen production

Aftercare — how to look after your result

Proper aftercare makes a difference. Here are our recommendations for the first few days after treatment:

The first 24 hours:

  • Avoid touching the treated areas (minimise the risk of bacteria)
  • No intense exercise — perspiration and increased blood circulation can worsen swelling
  • Avoid alcohol — can increase the risk of bruising and swelling
  • Sleep with your head elevated — reduces swelling, especially after lip treatment
  • Apply gentle cooling — an ice pack wrapped in cloth can relieve swelling

The first 1–2 weeks:

  • Avoid extreme heat — saunas, steam baths, and intense sun exposure
  • Be cautious with facial treatments — massage and pressure can displace the filler before it has stabilised
  • Avoid dental appointments if you have had lip treatment (if possible, wait 2 weeks)

Long-term care:

  • Sun protection — SPF 50 daily protects the skin and extends the longevity of your result
  • Moisturising — A good skincare routine supports the aesthetic result
  • Follow-up visit — Book a follow-up after 2–3 weeks for evaluation

Dissolving fillers — hyaluronidase

One of the greatest advantages of hyaluronic acid-based fillers is that they are fully reversible. If for any reason you are not satisfied with the result — or if a complication arises — the filler can be dissolved with the enzyme hyaluronidase.

How does it work?

Hyaluronidase is a naturally occurring enzyme that breaks down hyaluronic acid. By injecting the enzyme directly into the treated area, the filler is dissolved, usually within 24–48 hours. The process is:

  1. Consultation — We assess the reason and plan the dissolution
  2. Allergy test — In some cases, we perform a skin prick test to rule out an allergic reaction
  3. Injection — Hyaluronidase is injected into the relevant area
  4. Follow-up — We follow up after 1–2 weeks

Common reasons for dissolution

  • Dissatisfaction with the result (too much, asymmetry)
  • Filler migration — the product has shifted from its original placement
  • Complications — in rare cases, vascular compromise
  • A desire to "start over" with a new treatment

We see dissolution as a natural part of the toolkit — not as a failure. Having this option provides an extra dimension of reassurance.


Combination treatments — synergy for the best results

Important note: Combination treatments are always planned with sufficient intervals between each procedure. We never recommend multiple invasive treatments at the same session without medical justification.


Common concerns, myths, and misconceptions

"I'm afraid it will be obvious that I've had something done"

This is the most common concern we hear — and the most unfounded. Modern fillers, properly applied by an experienced injector, produce a result that looks like you, just a little better. The extreme results sometimes seen on social media are almost always the consequence of overtreatment or incorrect technique. Our philosophy is that the people around you should think you look rested and refreshed — not that they should be guessing what you have had done.

"Fillers stretch out the skin — it gets worse when they wear off"

Myth. Hyaluronic acid is broken down gradually by the body, and the skin returns to its original state. You do not end up "worse" than before — you return to your baseline. Moreover, studies show that repeated filler treatments can stimulate collagen production, which means the skin may actually be in better condition after the filler has been metabolised.

"Fillers are addictive"

There is no chemical dependency mechanism in fillers. What can happen, however, is that you appreciate your result and want to maintain it — just as you have regular haircuts without calling it an addiction. The most important thing is to have an injector who sets boundaries and recommends what is medically and aesthetically appropriate.

"Cheaper fillers work just as well"

Absolutely not. Product quality plays an enormous role in both results and safety. Cheap, non-CE-marked fillers may contain impurities, have unpredictable longevity, and spread in undesirable ways. We never compromise on product quality — it is a matter of your health and safety.

"You can't look natural with lip fillers"

You absolutely can look natural — it is all about volume, placement, and product selection. The key is to respect your natural lip shape and build upon it rather than trying to create an entirely new shape. "Fillers stop you from ageing naturally"

Fillers do not replace good skincare, sun protection, or a healthy lifestyle. They are a complement — a tool in the arsenal. The best results are achieved in combination with a well-considered skincare routine, sun protection, and regular skincare treatments.


Who should avoid fillers?

Filler treatment is contraindicated in the following cases:

  • Pregnancy and breastfeeding
  • Active infection in or near the treatment area (herpes, acne, skin infections)
  • Autoimmune diseases — discussed on an individual basis during consultation
  • Allergy to hyaluronic acid or lidocaine
  • Blood-thinning medication — requires special planning (always inform us at consultation)
  • Under 18 years of age
